Health Insurance
The City's health insurance is provided as a self-funded program in a Preferred Provider Network (PPO). To find out more information on our health insurance plan, select from the following choices:
- Coresource- Coresource is the City's Third Party Administrator (TPA), responsible for bill payment.
- Caremark- Caremark is the City's provider for prescriptions, including mail order.
- Medical Mutual of Ohio -Medical Mutual of Ohio is the city's preferred provider network.

Life Insurance
The City's life insurance is provided through Hartford Life and Accident Insurance Company. To view the policy, select the following link to the Booklet-certificate.
